Subcision Treatment for Scar Removal

What is Subcision for Scars?

Subcision is a minimally invasive surgical procedure used to improve depressed scars. At the Laser & Dermatologic Surgery Center, we feel the procedure is quite simple and very effective. One reason you can see depressed scars is that fibrous bands tack the skin inward. During subcision, a small needle is used to cut the fibrous bands from inside. This allows the skin to be lifted since it is not bound down anymore.

Which scars respond best to subcision?

The subcision technique works best for rolling acne scars and boxcar acne scars. Subcision will not work for ice pick acne scars. The CROSS technique is the gold standard treatment for ice pick scars. In this technique, TCA 80% is applied sparingly in the ice pick scar to induce collagen production within the scar tissue.

What can I expect during subcision?

The Laser & Dermatologic Surgery Center is dedicated to providing care in a safe and caring manner. After local anesthesia, Dr. Hruza or Dr. Behshad inserts a needle under the depressed scar. Then, using a fanning motion in the horizontal direction, the fibrous bands are severed. Occasionally, we will inject dermal filler into the indented space to improve the results of the subcision. The procedure takes no longer than 15-20 minutes and has minimal discomfort associated with it.  Multiple sessions are required, usually spaced approximately one month apart. The results will be better if subcision is combined with other acne scar treatments, such as fractional laser resurfacing, fillers, and chemical peels, in our St. Louis office.

What are side effects?

The most common side effects include bruising of the skin. Rare side effects can include hitting a blood vessel or a nerve. It is very important that a professional trained dermatologist such as Dr. Hruza or Dr. Behshad performs this procedure to avoid complications.

What can I expect after treatment?

Patients can experience bruising, swelling, and redness for a several days following the procedure. The 18 gauge needle is so small it does not leave a permanent mark. The procedure can be repeated to enhance the benefits and can be combined with lasers such as FRAXEL Restore: Dual or CO2RE, or with fillers such as Restylane®, Juvederm®, Perlane®, or Radiesse®.
